Certificate of Insurance

A certificate of insurance is a document that shows the named insured’s coverage. It states the effective and expiration dates, the carrier, the policy number and certifies the insured had coverage at the time the certificate was issued.

To request a certificate of insurance, fill out the form below. Temporary staffing or leasing or Professional Employment Organization (PEO) companies must request certificates of insurance by calling (405) 232-7663, ext. 5102 or (800) 347-3863, ext. 5102.
